Details of HQP’s data processing activities/Privacy Notice
We refer to the Partnership Agreement entered between you and Hospice Quality Partnership (“HQP”) (the “Agreement”). HQP is committed to protecting personal data provided by you, your employees and other individuals authorised to act on your behalf in accordance with UK data protection law. This letter explains what information we may collect, how and when we will use it and identifies your rights. This information is not exhaustive, and we are always happy to provide additional information or explanations where needed us. Please contact us by email, phone or in writing using the details at the end of this notice.


1. The information we collect.
We may collect information about your employees, representatives and other individuals authorised to act on your behalf (the “Individuals”). We may also on occasion collect information about your consultants. The information we collect or obtain will include names, business address, email addresses and telephone numbers. We may obtain this information face to face, electronically, on paper (such as any form you complete) or verbally (during any telephone conversations we have with you). Our records may be held on paper or electronic computer systems.


2. How we may use this information.
We, our suppliers, business partners and our sub-contractors, may use any of the information set out above for the purposes of:
(a) complying with our obligations under the Members’ Partnership Agreement.
(b) providing you with any agreed services/products.
(c) providing you with information on products and services offered by our suppliers, business partners or sub-contractors.
(d) for business management purposes and keep you informed of HQP activities

3. Disclosure of your information.
We may disclose the personal information set out above:
(a) to our agents, consultants and sub-contractors who assist us in running our business, or provide related services (including third party companies that conduct statistical analysis), and who are subject to appropriate security and confidentiality obligations;
(b) to our approved suppliers who may contact the Individuals directly in relation to marketing information;
(c) to senior management on a confidential basis for our internal administrative and other business purposes; and
(d) in the event that the whole or a substantial part of our business is sold or integrated with another business, to our advisers and any prospective purchasers (and their advisers).

We will also make whatever disclosures which are required by law. We will not sell any personal data collected hereunder to third parties.
In line with UK GDPR, HQP is a Data Controller and we are legally responsible for ensuring that all personal confidential data that we process i.e. hold, obtain, record use or share about you is done in accordance with data protection legislation. As a Data Controller we are registered with the Information Commissioner’s Office (ICO). All staff are trained to understand their responsibilities with respect to handling your data and responding to concerns or incidents.


4. Keeping information about you secure.
We will take appropriate administrative and technical measures to keep your information confidential and secure in accordance with our internal procedures covering the storage, access and disclosure of information.

 

5. Your Consent.
HQP will liaise with all those individuals and consultants for whom you have provided personal data to gain their consent to receive newsletters, business information and marketing communications in relation to the processing activities set out in this notice. If any of these individuals wish to opt-out at any time, they should contact us to request this.
Please note that due to the nature of the Partnership’s work, should we receive such a request, we may seek confirmation from the CEO or HQP Champion regarding the removal of such data from our records and identifying an alternative contact.

6. Disclaimer.
HQP makes every effort to ensure that the information on this Website is accurate and upto-date. If errors are brought to our attention, we will try to correct them. However, we cannot accept responsibility for any loss, damage or inconvenience caused by reliance on material contained in this Website.

Certain links in our site, including hypertext links, will take you outside the Website. We will make reasonable efforts to ensure the content available on linked websites is not defamatory or offensive. • it is not used in a derogatory or misleading context.
• HQP is acknowledged as the owner.
The permission to reproduce the Copyright Protected Material does not extend to copyrighted works, which are identified as being the copyright of a third party.

HQP endeavours always to make the Website as accessible as reasonably possible to all users. In no way does HQP seek to deliberately discriminate against disabled users to access the website service in accordance with the Disability Discrimination Act 1995.

h) Rights in relation to automated decision making and profiling (HQP does not use automated decision making and profiling).
